Routine NHS physiotherapy waits in Dorset commonly run to several months and vary by provider and clinical priority. For anything urgent the NHS is still the right route and that is not in question. For an ordinary back or shoulder, that is a long time to keep doing less than you would like to.

No GP referral is needed. If the assessment turns up something that belongs with a GP or a consultant, you will be told that plainly rather than sold a course of treatment.
